Why are children still leaving primary school unable to read fluently?

Could it be possible that we are not oriented enough towards the sciences of teaching and learning, together with a very extended curriculum that may include too much focus on events without stronger focus and discipline on and for, learning. Here I am thinking of discipline as it applies to practice to get better and consistently improve, not brutality.
